This form may be used by an employee to request leave under the Family and Medical Leave Act (FMLA). This form accommodates a new type of leave that can be used by an employee in the event they need to stay home in order to care for their child whose school or child care facility has been closed due to coronavirus disease 2019 (COVID-19).
